Kindred Hospital Chattanooga is seeking a dedicated care coordination professional to oversee patient pathways from admission through discharge. You will collaborate with the Interdisciplinary Care Transitions team, ensuring efficient resource use and adherence to care plans.

The role requires strong communication, critical thinking, and 2+ years in healthcare, with experience in case management or utilization review preferred. Fluency in English and reliable attendance are essential.

Qualifications

• Must read, write and speak fluent English.
• 2+ years experience in healthcare setting.
• Prior experience in case management, utilization review, or discharge planning (Preferred).

Responsibilities

• Coordinates clinical and psycho-social activities with the Interdisciplinary Team and Physicians.
• Monitors patient stay for effective care coordination and efficient care facilitation.
• Keeps current on reimbursement, community resources, case management, psychosocial and legal issues.
• Refers high risk patients for additional support.
• Acts as patient advocate to help families make informed decisions.
• Collaborates with care team to review goals and plan of care.
• Plans care across the continuum with post-acute providers and payors.
