Job summary

Kindred Hospital Chattanooga is seeking a qualified Care Coordination professional to oversee patient care across the continuum, collaborating with the ICT team and treating physicians to optimize outcomes and resource use.

The role focuses on care planning, implementation, and evaluation, with emphasis on patient advocacy and engagement, ensuring comprehensive discharge planning and coordination with post-acute providers.

Qualifications

• Must have RN licensure in state or eligible (or Compact State).
• Postsecondary nursing credential from an accredited program required.
• BSN/MSN/BSW/MSW is preferred.
• 2+ years healthcare experience required; case management/utilization review/discharge planning preferred.

Responsibilities

• Coordinate clinical/psycho-social activities with Interdisciplinary Team and Physicians.
• Monitor patient stay to ensure efficient care coordination and resource use.
• Refer high-risk patients for additional support as needed.
• Act as patient advocate to support informed decisions by patients and families.
• Participate in care rounds to align goals, resources and discharge planning.
• Collaborate with team, physicians, patients, families, post-acute providers, payors, and others in planning continuing care.
