Qualifications

• Minimum two years of acute care experience required.
• Maintain current licensure in Alabama.
• Demonstrated ability to adapt in fast-paced environments.

Responsibilities

• Perform assessments/evaluations of patients for post-hospital care.
• Develop and implement post-acute care plans.
• Collaborate with medical and nursing staff to ensure continuity of care.
