Qualifications

• Knowledge of government and non-government payor practices, regulations, standards and reimbursement.
• Knowledge of Medicare benefits and insurance processes and contracts.
• Knowledge of accreditation standards and compliance requirements.
• Ability to demonstrate critical thinking, prioritization and time management.
• Must read, write and speak fluent English.

Responsibilities

• Coordinates and facilitates care with Interdisciplinary Care Transitions (ICT) team members.
• Follows patients through continuum of care and ensures optimal resource utilization.
• Provides assessment, care planning, plan implementation and evaluation of patient needs.
• Enhances patient management and satisfaction through discharge planning and collaboration.
• Participates in rounds and conferences to review goals and maximize resource use.
