Pyramid Consulting, Inc. in Sanford/Sun City Center, FL is seeking a Director of Case Management for a full-time onsite leadership role.

You will oversee the Case Management department and coordinate effective care planning and discharge processes to support optimal patient outcomes. The ideal candidate has RN or LCSW credentials with at least two years of acute care leadership experience, a Bachelor's degree, and a preferred Master's in Nursing, Healthcare Administration, or Business

Qualifications

• Minimum 2 years of acute care Case Management leadership experience.
• Bachelor's degree required.
• Active RN or LCSW credentials; RN preferred.
• Master's degree in Nursing, Healthcare Administration, or Business Administration preferred.

Responsabilités

• Lead and oversee daily Case Management department operations.
• Supervise staffing, onboarding, and staff development.
• Ensure care coordination, discharge planning, and timely documentation.
• Monitor performance and drive quality improvement initiatives.
• Ensure compliance with policies and regulatory requirements.
• Collaborate with executive leadership and division Case Management.
• Drive productivity, resource utilization, and patient outcomes.
