PACS Inc is seeking a part-time team member to assist in planning and conducting activities for residents in a long-term care facility. Responsibilities include developing activity schedules, ensuring communication among staff and residents, and maintaining a clean activity department. Ideal candidates will have a high school diploma and experience in a similar setting.
Join our team! We are looking for someone part-time / on-call. This position requires availability during evenings, weekends, and days.
Pay rate is $18.00 / hour
Responsibilities include participating in planning and conducting individual, small, and large group activities. Assist in maintaining effective communication between employees at all levels, residents, their families, support personnel, government agencies, and the public to meet the needs and interests of residents, the community, and the facility. Contribute to community planning related to the facility's services and residents' needs. Help develop the monthly activity calendar and maintain attendance records. Support the Quality Assurance and Assessment Committee in developing and implementing plans to address quality deficiencies as directed by the Activities Director.
Assist with discharge planning, developing and implementing activity care plans, and conducting resident assessments. Help arrange transportation for residents for outings, appointments, or discharges. Develop a monthly activity schedule that includes outings, group activities, and in-room activities for bed-bound or isolated residents. Encourage residents to participate in hobbies, crafts, reading, and other self-initiated activities, providing materials like Braille or audiobooks as needed. Support documentation efforts, including quarterly progress notes, as assigned by the Activities Director. Maintain the cleanliness, orderliness, and security of the Activity Department.
Supervisory Requirements: This position has no supervisory responsibilities.
Qualifications: High school diploma or equivalent required. Preferably, one year of experience in a long-term care facility.
Language Skills: Ability to read technical procedures, understand policy and procedure manuals, and effectively communicate with managers and staff.
Mathematical Skills: Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
Reasoning Ability: Capable of solving practical problems and interpreting instructions in various formats.
Physical Demands: The role requires standing and walking frequently, sitting occasionally, reaching, pushing, pulling, talking, hearing, tasting, smelling, and lifting up to 50 pounds. Occasional climbing, balancing, stooping, kneeling, crouching, or crawling. Good vision, including the ability to adjust focus.
Work Environment: Typically low to moderate noise levels. Reasonable accommodations can be provided for individuals with disabilities.
