Job summary

Forest Hills Care & Rehab in Tulsa, OK is seeking an Activities Assistant for weekend coverage. You will plan and lead floor activities, prep snacks, set up dining spaces, and assist residents to common areas according to care plans.

The role includes charting logs, supporting other departments, and participating in events. Qualifications include a High School Diploma or GED, NCCAP credits preferred, and OTA or NCCAP certification preferred. Weekend availability is required.

Qualifications

• High School Diploma or GED required.
• 6 NCCAP credits preferred.
• 500 hours of experience in activity programming preferred.
• OTA or NCCAP Certification preferred.

Responsibilities

• Schedule, pre-plan and implement floor activities based on center activity schedule.
• Use monthly calendar ideas to plan and organize special floor activities.
• Prepare special snacks with residents.
• Set up the dining room tables for activities.
• Clean up after all activities.
• Complete required logs and charting.
• Keep activity areas clean, safe and free from clutter.
• Assist residents to and from common areas as stated by their care plan.
• Assist other departments with residents as needed and required.
• Weekends, holidays and evenings required.

Education

High School Diploma or GED

6 NCCAP credits preferred

500 hours of activity programming experience preferred

OTA or NCCAP Certification preferred

• Every Saturday & Sunday: 8:30am-5pm

Qualificationsof Activities Assistant:

• High School Diploma or general equivalency is required

• 6 college credits in applicable study as required by NCCAP is preferred

• 500 hours of experience in activity programming is preferred

• Certification as Occupational Therapy Assistant or NCCAP Certification is preferred
