Job summary

Denton MHMR is seeking an ACT Team Lead to coordinate case management and rehabilitation services for individuals with severe mental illness. The role involves leading meetings, developing recovery plans, and ensuring adherence to HHSC guidelines.

You will work under supervision, with responsibilities spanning service delivery, documentation, and collaboration with community partners to support recovery and community integration.

Qualifications

• Graduation from an accredited four-year college or university in a human services field; QMHP preferred and two years in mental health with six months as case manager.
• Master's Degree in human services preferred; community/crisis experience valued.

Responsibilities

• Attend and participate in all scheduled meetings, including daily ACT team meetings.
• Provide case management and rehabilitation services in a team-based environment.
• Develop Recovery Plans and coordinate Recovery Plan meetings.
• Facilitate treatment groups and provide transportation as needed.
• Ensure documentation is complete and compliant with HHSC guidelines.
• Assist with supervision of ACT staff and coordinate with community partners.
