Job summary

Postgraduate-Center-For-Mental-Heal is seeking an experienced leader to supervise a multidisciplinary ACT team delivering clinical and case management services to adults with serious mental illness. You will oversee admissions, treatment planning, and discharge processes, while ensuring quality data and compliance with NYS requirements.

The role emphasizes evidence-based practices, team leadership, and collaboration across agency staff to meet contract objectives.

Qualifications

• Master’s degree and NYS licensure as LMHC/LMSW/LCSW or equivalent.
• Minimum two years of experience in human services with serious mental illness.
• Experience with co-occurring substance use disorders preferred.
• Proficiency in Microsoft Office Suite (Word/Outlook/Excel).

Responsibilities

• Supervise clinical and case management services by a multidisciplinary team.
• Direct admission, treatment, rehabilitation, and discharge processes.
• Lead treatment planning meetings and monitor workflow and staff performance.
• Prepare weekly reports and census projections for SPOA.
• Coordinate with MCOs for service authorizations and on-call coverage.
