Job summary

Family Health Services is seeking a full-time clinician to work with the Assertive Community Treatment (ACT) multidisciplinary team to help patients achieve recovery goals. You will use skills-building techniques and trauma-informed care to support individuals with co-occurring SPMI and SUD, in-office and community settings, including driving to locations as needed.

The role requires a minimum bachelor’s degree in a human service field; LSW is preferred.

Qualifications

• Minimum bachelor’s degree in a human service related field.
• LSW preferred.
• Experience working with people who have SPMI and co-occurring SUD/MH diagnoses.
• Ability to use skill-building techniques (role-play, modeling, reinforcement, coaching, shaping, fading, bibliotherapy).

Responsibilities

• Assist patients in identifying their strengths and assets to promote recovery and resilience.
• Develop skills-building related treatment goals and interventions.
• Provide trauma-informed care focus and psycho-education with individuals, families and groups.
• Consult with ACT team members to help patients achieve recovery goals.
• Assist patients in identifying and engaging with community resources and supports.
• Establish and maintain case files, referrals, and related documents.
• May supervise activities of staff and interns.
• Participate in supervision meetings and may plan outreach programs.
