Job summary

Family Health Services Foundation is seeking a full-time clinician to work with the Assertive Community Treatment (ACT) multidisciplinary team. You will use skills-building approaches to support patients with SPMI and SUD, in office and community settings, including on-call and weekend hours.

Emphasis on trauma-informed care, collaboration, and independent decision making. Qualifications include a minimum bachelor’s degree in a human services field and LSW preferred.

Qualifications

• Bachelor’s degree in a human service related field.
• LSW preferred.
• Experience working with people who have SPMI and co-occurring SUD/MH diagnoses.
• Proficient with computers and electronic charting in health records.

Responsibilities

• Assist patients in identifying strengths to promote recovery and resiliency.
• Develop skills-building related treatment goals and interventions.
• Provide trauma-informed care and psycho-education.
• Consult with ACT team to help patients achieve recovery goals.
• Assist patients in engaging with community resources and supports.
• Help patients overcome barriers to independence and practice skill-building.
• Establish and maintain case files, referrals, and related documents.
• May supervise activities of staff and interns; participate in supervision meetings.
