Job summary

Carolina Outreach is looking for an ACT Psych Rehab Specialist to join our Assertive Community Treatment (ACT) team. The role provides community-based, in-home interventions to clients with mental health and substance use challenges, focusing on symptom stability and independent living.

You will deliver person-centered counseling, psychoeducation, and behavioral supports, while coordinating with families, natural supports, and community resources.

Qualifications

• QP status per 10A NCAC 27G.0104 with a relevant bachelor degree and 2+ years MH/DD/SA post-bachelor experience, or SUDP with 2+ years post-bachelor in addiction counseling.
• Alternate: BA in non-human services with 4+ years post-bachelor MH/DD/SA experience.
• Previous MH/SA treatment experience preferred.

Responsibilities

• Provide community-based, in-home individual and family interventions for mental health and substance use recovery.
• Deliver counseling, psychoeducation, behavioral interventions, and treatment plan-based services.
• Collaborate with families, supports, and community partners to coordinate care.
• Maintain timely documentation and ensure ACT program standards compliance.
• Support families and teach skills to improve daily living and independence.
