Job summary

Family Health Services in Idaho is seeking a dedicated team member to join the ACT multidisciplinary group, helping patients achieve recovery goals through skills-based interventions. The role blends office and community visits, with protected 40-hour weeks and shared on-call duties.

You will use recovery-focused coaching, role-play and behavioral strategies to support individuals with SPMI and co-occurring SUD/MH conditions. Travel between sites is required, and EHR documentation is essential.

Qualifications

• Minimum bachelor’s degree in a human service related field.
• LSW preferred.
• Experience working with people who have SPMI and co-occurring SUD/MH diagnoses.
• Ability to use skill-building techniques and psychoeducation.

Responsibilities

• Assist patients in identifying their strengths and assets to promote recovery.
• Develop goals and interventions related to skills building.
• Provide trauma-informed care and psycho-education with individuals, families and groups.
• Consult with ACT team members to help patients achieve recovery goals.
• Engage with community resources and supports for patients.
