Job summary

Family Health Services is seeking candidates to join the Assertive Community Treatment (ACT) team. This full-time role supports patients with co-occurring SPMI and SUD, using skills-building, role-play, and coaching to promote recovery. Work is in office and community settings with on-call and weekend hours.

Requires a Bachelor's in a human service field; LSW preferred. EHR proficiency and strong collaboration with a multidisciplinary team are essential. A quarterly bonus is available.

Qualifications

• Bachelor’s degree in a human service field; LSW preferred.
• Experience with SPMI and co-occurring SUD/MH diagnoses.
• Ability to use skill-building techniques such as role-play, modeling, reinforcement, coaching.
• Proficient in computers; EHR use required.
• Ability to work independently and in a diverse team.

Responsibilities

• Assist patients in identifying strengths and assets to promote recovery.
• Develop skills-building related treatment goals and interventions.
• Provide trauma-informed care and psycho-education.
• Consult with ACT team to help patient achieve recovery goals.
• Engage with community resources and supports for patients.
• Help patients overcome barriers to independence and practice skills.
• Maintain case files, referrals, and related documents.
• May provide teaching or training to professionals when needed.
• Participate in supervision meetings with supervisor.
• Plan outreach and education programs and activities.
• May supervise interns or staff for identified program.
• Perform miscellaneous duties as assigned.
