Job summary

St. Luke's Health Network, Inc. seeks an ACT/FACT Team Leader to coordinate a multidisciplinary Assertive Community Treatment Team and supervise services ensuring fidelity to ACT standards and licensing requirements.

The role involves direct client services in community settings, supervision of staff, and collaboration with HR and program managers to ensure quality care and compliance across all activities.

Qualifications

• Master's degree in social work, psychology, counseling, or related license-eligible field.
• Professional Pennsylvania Clinical License required (LCSW, LPC, LMFT, etc.).
• Minimum of two years of experience working with ACT/FACT teams or equivalent supervision experience.

Responsibilities

• Directs the day-to-day clinical operation of the ACT Team, including scheduling staff across day, evening, weekend, and holiday shifts.
• Leads daily organizational staff meetings and treatment planning meetings.
• Maintains ACT Team census and coordinates admissions, assessments, and treatment planning.
• Provides ongoing supervision to all ACT staff and ensures timely, accurate documentation.
• Oversees recruitment, interviewing, onboarding, and termination in coordination with HR and Network leadership.
• Delivers in vivo services to ACT participants in homes and community settings to improve quality of life and community tenure.
