Job summary

InterCommunity, Inc. is a Federally Qualified Health Center Look‑Alike (FQHC LA) delivering accessible primary and behavioral health services at sites in East Hartford, Hartford, and South Windsor.

The ACT Community Support Specialist will work in clients' homes and community settings as part of the ACTT to build skills, coordinate care, and promote recovery. The role requires a bachelor’s or associate degree in health or behavioral health, strong communication, and teamwork.

Qualifications

• Strong communication and collaboration within a transdisciplinary ACTT team.
• Ability to link clients to clinical, medical, social, educational, rehabilitative and vocational services.
• Proficiency in documenting treatment activities and scheduling.

Responsibilities

• Active participation on a transdisciplinary ACTT team with daily reviews.
• Link clients to needed clinical, medical, social and other services.
• Assess and identify required services with clients and team.
• Support recovery through individual treatment sessions or external providers.
• Complete documentation including court letters, records, and scheduling.
• Engage clients and track attendance with reports.
• Create and facilitate client-centered groups in office and community settings.
• Assist in crisis assessment and detox admission decisions.
• Educate clients and families to support wellbeing.
• Perform related duties as required.
