Job summary

Heritage Behavioral Health Center in Decatur, Illinois is hiring a Community Support Specialist for the Assertive Community Treatment (ACT) team. You will deliver intensive, community-based mental health services to adults living with serious mental illness, primarily outside the office, in homes and community settings.

Qualifications

• High School Diploma or GED + 5 years of supervised mental health or social service experience
• Bachelor’s degree in social work, psychology, education, or a human service field preferred
• Minimum 21 years of age
• Valid driver’s license, reliable transportation, and proof of auto insurance

Responsibilities

• Provide intensive, community-based mental health services to adults living with serious mental illness
• Engage clients in person and by phone in community settings
• Deliver case management and recovery-focused supports using ACT principles
