Job summary
Community Partners, NH is a nonprofit in Strafford County delivering developmental and mental health services. The ACT Functional Support Specialist will provide recovery-focused counseling and connect clients with community resources in home-based and community settings.
You’ll work with adults with serious mental illness and their families, using technology to meet job requirements. Qualified candidates hold a Bachelor’s degree in Human Services or related fields, have excellent communication
Qualifications
- Bachelor’s degree in Human Services or related field is required.
- Experience with adults with mental health issues and families preferred.
- Excellent oral and written communication skills.
- Ability to utilize technology provided to perform job requirements.
- Holds a valid driver’s license and proof of vehicle liability coverage.
Responsibilities
- Provide recovery-focused community and home-based counseling and therapeutic services.
- Link to and consult with community resources for adults with serious mental illness.
