InterCommunity, Inc. in East Hartford, CT is hiring a Community Support Specialist to deliver care coordination in home and community settings. You will work with clients in recovery, link them to resources, and document treatment per procedures.

The role requires collaboration with clinical teams, participation in meetings, and driving client engagement while supporting daily operations in a community health center environment.

Responsabilidades

• Collaborate with the clinical team to identify required services by assessing client needs.
• Support client recovery by joining OP clinicians, Primary Care and participating in sessions.
• Provide linkages with community resources and complete treatment documentation per procedures.
• Engage clients and support attendance by completing tracking documents and reports.
• Assist during crisis assessments and determine need for detox and admission to detox facilities.
• Provide education and support to families and support networks.
• Perform related duties including transportation of clients; must have reliable insured vehicle.
• Attend mandatory department and agency meetings and trainings.

Educación

Bachelor’s or Associate degree in health/behavioral health
