Job summary

Holton Manor in Elkhorn, WI is seeking a licensed practical nurse (LPN) to provide compassionate nursing care under supervision according to care plans and physician orders. This on-site role offers flexible scheduling and a chance to work with a dedicated health care team.

You will administer medications, monitor residents, document in the EMR, mentor CNAs, ensure infection control, and participate in ongoing training.

Qualifications

• Active LPN license in Wisconsin or eligible for reciprocity.
• CPR certification.
• Ability to meet physical and job-related requirements.
• Must be fluent in English with strong written and verbal communication skills.

Responsibilities

• Provide safe, effective nursing care per care plans and physician orders.
• Administer medications and treatments accurately.
• Assess residents and report changes promptly.
• Document care in the EMR.
• Mentor CNAs and monitor performance.
• Ensure infection control and safety compliance.
• Complete all other duties as assigned.

Job Summary:

TheLPNmust beaclinically competent, licensed practical nurse who provides care to a group of patients/residents under general supervision/delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with patients/residents, families, responsibleparty, and all health team members.

Preferred:

• Long-term care or skilled nursing experience
