Holton Manor in Elkhorn, Wisconsin, is seeking a Licensed Practical Nurse (LPN) to join our 60-bed skilled nursing community. On-site position with flexible scheduling to fit your needs, and multiple status options (Full-Time, Part-Time, PRN) available.

You will provide safe nursing care, administer medications, and collaborate with the health team under RN supervision. We offer comprehensive benefits including health, dental, vision, life insurance, 401(k) with employer match, PTO, flexible

Qualifications

• Active LPN license in Wisconsin or eligible for Wisconsin licensure.
• CPR certification.
• Ability to meet physical and job-related requirements.
• Excellent written and verbal communication in English.

Responsibilities

• Provide safe, effective nursing care per care plans and physician orders.
• Administer medications and treatments accurately.
• Assess residents and report changes promptly.
• Document care in the EMR.
• Mentor CNAs and monitor performance.
• Ensure infection control and safety compliance.
• Complete all other duties as assigned.

Job Summary:

TheLPNmust beaclinically competent, licensed practical nurse who provides care to a group of patients/residents under general supervision/delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with patients/residents, families, responsibleparty, and all health team members.

Key Responsibilities:

Qualifications:

Required:

• ActiveLPN licenseinstate of Wisconsin orlicensedin any U.S state and be eligible for Wisconsin licensure by reciprocity.(in good standing)

• Must be English literate andpossessexcellent writtenandverbal communication skills

Preferred:

• Long-term care or skilled nursing experience
• Clinical experience
