Holton Manor in Elkhorn, WI, is seeking a Licensed Practical Nurse (LPN) to provide clinical care for a resident group under supervision of a registered nurse. The role includes collaboration with families and health team members to ensure quality care in a on-site setting.

The LPN must be licensed and CPR-certified, with strong communication skills and the ability to handle physical requirements. Flexible scheduling options are offered across full-time, part-time, and PRN statuses.

Qualifications

• Active LPN license in Wisconsin or reciprocity (in good standing).
• CPR certification required.

Responsibilities

• Provide safe, effective nursing care per care plans and physician orders.
• Administer medications and treatments accurately.
• Assess residents and report changes promptly.
• Document care in the EMR.
• Monitor performance and mentor CNAs.
• Ensure infection control and safety compliance.
• Complete other duties as assigned.

Job Summary:

The LPN must be clinically competent, licensed practical nurse who provides care to a group of patients/residents under general supervision/delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with patients/residents, families, responsibleparty, and all health team members.

Required:
