Holton Manor in Elkhorn, WI seeks a compassionate Certified Nursing Assistant (CNA) to provide patient/resident care under RN/LPN supervision. You will perform data collection, follow the care plan, and ensure timely, safe care with clear communication to families and the health team.

The role emphasizes accuracy, time management, and adherence to policies, with opportunities for continuing education and a supportive team culture.

Qualifications

• Must be in good standing on the State Nursing Assistant registry or appropriate entity.
• Ability to meet physical and job-related requirements.
• Must be English literate and possess excellent written and verbal communication skills.
• Preferred: Previous hospital or long-term care or skilled nursing experience.
• CPR Certified.

Responsibilities

• Performs initial and ongoing data collection on all patients/residents under the direction of the Registered Nurse/Licensed Practical Nurse.
• Implements plan of care developed by the Registered Nurse based on patient/resident physical, psychosocial, cultural, ethnic and religious/spiritual needs.
• Provides direct patient/resident care and reports condition and changes to RN or LPN.
• Demonstrates time management and prioritization of tasks.
• Documents results of patient/resident care within established policies, procedures and scope of practice.
• Nutritional care.
• Effective communication with residents and family.
• Completes all other duties as assigned.

Job Summary:

TheCertified Nursing Assistant (CNA) will provide patient/resident care as required under the direct supervision of the licensed nurse.TheCNA at Holton Manorcompletes assigned tasks to ensuretimely,efficientandsafe patient/resident care. CNAbase hourly wage:$19.00 – $22.28per hourdependenton experience

PLUSshift differentials- $2.00 per hour PMshiftand $2.50 per hour NOC shift
