Job summary

Holton Manor in Elkhorn, WI is seeking a licensed practical nurse (LPN) to join our 60-bed skilled nursing community on-site. You will provide safe, competent nursing care under supervision and collaborate with RNs, CNAs, residents, families, and health team members.

Responsibilities include administering medications per orders, assessing residents, documenting in the EMR, maintaining infection control, and mentoring CNAs.

Qualifications

• Active LPN license in Wisconsin or eligible for Wisconsin licensure by reciprocity.
• CPR certified.
• Ability to meet physical and job-related requirements.
• English literate with excellent written and verbal communication skills.

• Provide safe, effective nursing care per care plans and physician orders.
• Administer medications and treatments accurately.
• Assess residents and report changes promptly.
• Document care in the EMR.
• Monitor performance and mentor CNAs.
• Ensure infection control and safety compliance.
• Complete all other duties as assigned.

Job Summary:

TheLPNmust beaclinically competent, licensed practical nurse who provides care to a group of patients/residents under general supervision/delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with patients/residents, families, responsibleparty, and all health team members.
