Job summary

Fraser in Bloomington, MN seeks a Group Home Coordinator to oversee day-to-day operations at our group homes, providing leadership, coaching, and staff training. You will ensure Fraser policies and 245D compliance while supporting quality improvement and a safe, supportive environment for clients and staff.

In this hands-on role you’ll work directly with individuals to support independence and daily living activities.

Qualifications

  • HS Diploma or equivalent with 4 years of direct care experience.
  • BA/BS in Human Services or related field with 1 year direct care experience.
  • Associate degree in Human Services or related field with 2 years direct care experience.

Responsibilities

  • Provide on-site supervision and leadership to staff at the group home or residential location.
  • Ensure compliance with 245D regulations and Fraser policies.
  • Support active treatment service development and implementation.
  • Lead continuous quality improvement initiatives at the location.
  • Assure necessary staff coverage, and recommend staffing to meet client needs and Fraser budget.
  • Serve as a primary point of contact for clients, families, and community partners.
  • Monitor and maintain accurate client and staff records per regulations.
  • Participate in emergency response and preparedness activities.
  • Coordinate activities of daily living including personal care, hygiene and grooming.
  • Coordinate and/or complete all household repairs and upkeep.
  • Ensure required documentation for client care is completed (Health Progress Notes, forms, notes, data collection).
  • Coordinate admissions, discharges and grievances with leadership.
  • Collaborate with medical professionals to coordinate health/medical needs.

Fraser is seeking a Group Home Coordinator / Designated House Coordinator (DHC) to join our Community Living team. In this role, you will oversee the day-to-day operations of a group home while providing leadership, coaching, and training to staff. You will ensure compliance with Fraser policies and 245D regulations, support continuous quality improvement efforts, and help maintain a safe, supportive environment for both staff and the individuals we serve.

This hands‑on leadership position includes working directly with individuals living in our group homes to support their personal goals, promote independence, and assist with daily living activities and personal care needs as required. A portion of your weekly schedule will be dedicated to providing direct support services.

Responsibilities

  • Assure necessary staff coverage, and recommend staffing, to meet the needs of the clients served and the budgetary parameters of Fraser.
  • Serve as a primary point of contact for clients, families, and community partners.
  • Monitor and maintain accurate client and staff records, including documentation required by regulations.
  • Participate in emergency response and preparedness activities as needed.
  • Coordinate the activities of daily living including personal care, hygiene, and grooming.
  • Coordinate and/or complete all household repairs and other upkeep.
  • Assure required documentation for all areas of client care are completed as required, including but not limited to Health Progress Notes, appointment forms, narrative notes, and data collection.
  • Coordinate admissions, discharges and grievances, in collaboration with the Assistant Director and the Director.
  • Collaborate with medical professionals to coordinate health and medical needs for each client served.
Qualifications
  • HS Diploma or Equivalent AND four (4) years of full‑time work experience (8,000 hours) providing direct care services to persons with disabilities or nursing/assisted living home experience
  • BA/BS in Human Services or related field AND 1 year of full‑time work experience providing direct care services to persons with disabilities
  • Associate degree in Human Services or related field AND 2 years of full‑time work experience (or equivalent) providing direct care services to persons with disabilities

Location, schedule & pay
  • Location:
    • Bloomington, MN
    • Plymouth, MN
  • This position is full‑time Monday‑Friday, 40 hours/week
    • Periodic weekend on‑call shifts
  • The starting pay range for this role is $21.50 - $23.50 with an additional 2-dollar differential per hour depending on up qualifications and location
  • $1,000 Hiring Bonus for external applicants
