Fraser in Bloomington, MN is seeking a Group Home Coordinator / Designated House Coordinator (DHC) to lead our Community Living team. You will supervise staff, ensure 245D compliance, and drive continuous service improvement while directly supporting individuals in our group homes.
You will also provide hands-on care, help staff develop, and serve as a liaison with families and partners as Fraser carries out its mission.

Fraser is hiring a Group Home Coordinator / Designated House Coordinator (DHC) to join our Community Living team. In this role, you will guide and train staff, ensure compliance with Fraser policies and 245D regulations, and support continuous improvement in services. You will work directly with the individuals we serve within our group homes, helping them achieve their personal goals, maintain independence, and receive assistance with daily living activities and personal cares as needed. Responsibilities also include providing onsite staff training, monitoring service quality, and ensuring the home operates in alignment with Fraser's mission and standards. This is a hands-on leadership role, and a portion of your weekly schedule will include providing direct care and support to individuals receiving services.
Candidates must qualify based in one of the following three ways, as assigned by 245D, which includes a combination of work experience and education.
