Fraser in Minnesota seeks a Program Coordinator to join our Community Living team. You will provide leadership, supervision, and mentorship to staff while ensuring high-quality services aligned with 245D regulations and individual plans.
You'll oversee one Supportive Living apartment setting, coordinate with families and providers, develop staff training, and maintain compliant documentation, billing, and records.

Fraser is seeking a Program Coordinator to join our Community Living team! In this leadership role, you will support individuals served while providing guidance, supervision, and mentorship to staff to ensure high-quality services. You\'ll help create a positive team culture, support individualized treatment and service planning, and ensure compliance with 245D regulations.
Community Living includes both Supervised Living homes and Supportive Living apartments . This role will support one of Fraser’s five Supportive Living apartment settings , where residents live more independently and do not require 24/7 supervision. The Program Coordinator will oversee one apartment complex.
