Job summary

A healthcare organization in Singapore is seeking qualified candidates to conduct assessments and formulate care plans for individuals with dementia. Qualifications

Responsibilities
  • Conduct case assessment and formulate Individualized Care Plan for Persons with Dementia and their carers.
  • Provide intervention such as psychoeducation, supportive counselling and service referral. Able to administer Advance Care Planning will be a bonus.
  • Liaise and collaborate with multidisciplinary professionals and external service providers for holistic care management.
  • Provide case recordings and relevant documentations.
  • Participate in team meetings and case conferences as required.
  • Assist in the preparation of data and statistics for programme evaluation and reporting purposes.
  • Plan and facilitate Caregiver Support Group sessions.
  • Support in outreach events and talks.
  • Any ad-hoc tasks as and when required.
Requirements
  • Preferably with degree in an allied health profession/social work/ psychology etc. Senior position may be considered for this role.
  • Preferably 2 years of clinical experience.
  • Good oral, written and interpersonal communications skills.
  • Ability to facilitate group sessions will be an added advantage.
  • Able to work on occasional Saturdays and weekday evenings.
  • Passionate about dementia care and working with people.
  • Prior working experience in dementia care will be an added advantage.
