Job summary

Home Nursing Foundation seeks a dedicated professional to deliver psychosocial support, assessments, and case management to clients with mental health needs and dementia. The role involves home visits, caregiver education, and coordination with Social Workers.

The ideal candidate holds a relevant Diploma and 2–3 years in healthcare or community care, with strong communication and teamwork skills to support elderly patients in the community.

Qualifications

• Diploma in social work, counselling, psychology or related healthcare/community care field.
• Preferably 2–3 years of healthcare/community care experience.
• Experience in health and social services with interest in care for persons with mental health issues and caregivers.

Responsibilities

• Conduct initial assessment and follow-up with intervention plan per care plan discussed with Social Worker.
• Provide case management, home visits, caregiver support and psychoeducation to clients with mental health conditions and/or dementia and their caregivers.
• Provide psychosocial support through cognitive stimulation activities or reminiscence and other engaging activities based on clients' interests.
• Support planning and organization of structured preventive activities and programmes for the community at risk of mental health conditions.
• Monitor clients' conditions and progress, escalate to Social Worker when needed.
• Provide logistical or operational support for clients and caregivers.

Dedicated to all-round support for our patients, HNF delivers comprehensive care programmes to look after patients' emotional well-being and healthcare needs.Our HNF Wellness Day Care & Rehabilitation Centres offer an array of social activities, social support and holistic healthcare services to help our elderly stay independent in their own community.

RESPONSIBILITIES

• Conduct initial assessment, follow-up with intervention plan, as per the care plan as discussed with the Social Worker.


• Provide case management, home visits, caregiver support, and psychoeducation to clients with mental health conditions and/or dementia and their caregivers.


• Provide psychosocial support to clients and/or caregivers through engaging them in meaningful activities such as cognitive stimulation activities or structured activities e.g. reminiscence, memory, linguistic and other appropriate activities) based on clients' interest.


• Support in the planning and organization of structured preventive activities and/or programmes for the community, those at risk of mental health conditions and clients.


• Monitor clients' conditions and progress, and escalatate to the Social Worker for support.


• Provide logistical or operational support for clients and/or caregivers.


• Ensure attendance in training and supervision sessions, where relevant, as part of the staff competency required by the CREST programme.


• Network and engage with community partners such as grassroot leaders, Social Service Agencies (SSAs) and hospitals to raise their mental health awareness and understand services provided by CREST


• Provide support in mental health training and education to the public

Any other related duties as directed by the said reporting officers.

QUALIFICATIONS

• Diploma in social work, counselling, psychology, or other fields related to healthcare and community care



• Preferably 2-3 years of healthcare/community care experience



• Experience in the health and social services sector with specific interest to care for persons with mental health issues and their caregivers will be preferred




• Possess good interpersonal and communication skills; demonstrate understanding, and listening skills




• Good time management skills, well-organised, meticulous, efficient and able to multi-task




• Takes initiative and able to work well independently and as a team player
