Job summary

NTUC Health Co-operative Limited invites applications for a Case Executive to join the CREST-PDS team. You will provide case management support for clients diagnosed with dementia and their caregivers, coordinating care and linking services with the right resources.

You will participate in multidisciplinary case discussions, conduct home visits, and help evaluate and document programme outcomes while handling ad-hoc tasks from the CREST-PDS Lead.

Qualifications

• Degree or diploma in Social Work, Psychology or Counselling is required.

Responsibilities

• Provide case management support for clients newly diagnosed with dementia and caregivers.
• Coordinate care and link services with appropriate resources and caregiver support.
• Participate in multi-disciplinary discussions with hospital staff about client wellbeing.
• Conduct regular home visits to monitor caregiver and client wellbeing.
• Assist with programme evaluation and documentation.
• Support CREST-PDS Lead/Department Head with ad-hoc tasks.

Job Description

Join us as a Case Executive and be part of our CREST-PDS team.

You will need to:

• Provide case management support to clients who are newly diagnosed withdementia and their caregivers
• Provide care coordination and service linkage to clients with dementia and their caregivers, with the appropriate resources and support including caregiversupport
• Participate in multi-disciplinary case discussions with hospital professionalsrelating to the well-being of clients who are diagnosed with dementia
• Conduct regular home visits to monitor the well-being of the caregiver and clientwith dementia
• Support the evaluation and documentation of the programme
• Support other ad-hoc assignments as assigned by CREST-PDS Lead/Department Head
