Job summary

PEOPLE PROFILERS PTE. LTD. in Singapore is seeking a dedicated care coordinator to support clients with mental health conditions and dementia and their caregivers under the CREST programme.

You will conduct initial assessments, plan interventions, and provide case management including home visits. The role emphasizes psychosocial support, engagement in meaningful activities, coordination with social workers, and collaboration with community partners to raise awareness.

Qualifications

• Diploma in Social Work is required or strongly preferred.
• Experience in health and social services is preferred.

Responsibilities

• Conduct initial assessment and develop intervention plans with the Social Worker.
• Provide case management, home visits, caregiver support, and psychoeducation.
• Deliver psychosocial activities such as cognitive stimulation and reminiscence.
• Assist in planning preventive community activities for at-risk clients.
• Monitor client conditions and escalate when needed.
• Provide logistical and operational support for clients and caregivers.
• Attend CREST training and supervision sessions.
• Engage with community partners to raise mental health awareness.
• Deliver public mental health education.
• Perform other duties as directed.

Job description

• Conduct initial assessment, follow-up with intervention plan, as per the care plan as discussed with the Social Worker.

• Provide case management, home visits, caregiver support, and psychoeducation to clients with mental health conditions and/or dementia and their caregivers.

• Provide psychosocial support to clients and/or caregivers through engaging them in meaningful activities such as cognitive stimulation activities or structured activities e.g. reminiscence, memory, linguistic and other appropriate activities) based on clients' interest.

• Support in the planning and organization of structured preventive activities and/or programmes for the community, those at risk of mental health conditions and clients.

• Monitor clients' conditions and progress, and **escalate** to the Social Worker for support.

• Provide logistical or operational support for clients and/or caregivers.

• Ensure attendance in training and supervision sessions, where relevant, as part of the staff competency required by the CREST programme.

• Network and engage with community partners such as grassroot leaders, Social Service Agencies (SSAs) and hospitals to raise their mental health awareness and understand services provided by CREST

• Provide support in mental health training and education to the public

Any other related duties as directed by the said reporting officers.

Qualifications

• Minimum Diploma in Social Work

• Experience in the health and social services sector with specific interest to care for persons with mental health issues and their caregivers will be preferred

• Possess good interpersonal and communication skills; demonstrate understanding, and listening skills

• Good time management skills, well-organized, meticulous, efficient and able to multi-task

• Takes initiative and able to work well independently and as a team player
