NTUC Health Co-operative Limited (NTUC Health) in Singapore is seeking a Senior Social Worker to deliver comprehensive psychosocial assessment, intervention, and case management for clients with complex needs within the community. Qualifications

  • Degree or postgraduate qualification in Social Work recognised by SASW.
  • Registered Social Worker (RSW) preferred.
  • Minimum 5–8 years of relevant social work experience, preferably in healthcare/e eldercare.
  • Experience managing complex cases and working with vulnerable populations.
  • Supervisory or mentoring experience preferred.

  • Develop and implement person-centred care plans based on clients' needs and goals.
  • Provide counselling, emotional support, crisis intervention and practical assistance to clients and families.

Job Description

The Senior Social Worker is responsible for providing comprehensive psychosocial assessment, intervention, and case management support to clients with complex social and healthcare needs within the community. The role supports clients and caregivers to age-in-place safely and with dignity through coordination of care, advocacy, counselling, and multidisciplinary collaboration.

You will need to:

Casework and Case Management
  • Conduct comprehensive psychosocial assessments for clients and caregivers.
  • Develop and implement person‑centred care plans based on clients' needs and goals.
  • Provide counselling, emotional support, crisis intervention and practical assistance to clients and families.
  • Manage complex and high‑risk cases independently.
  • Monitor clients' progress and review intervention plans regularly.
  • Facilitate referrals and coordinate services with hospitals, community partners, government agencies and healthcare providers.
  • Advocate for clients' needs and access to community resources and financial assistance schemes.
Caregiver and Family Support
  • Support caregivers in coping with caregiving stress and challenges.
  • Conduct family meetings and mediate family related issues where necessary.
  • Educate clients and families on available social and healthcare resources.
Multidisciplinary Collaboration
  • Work closely with nurses, doctors, therapists, care coordinators and community partners to ensure integrated care delivery.
  • Participate in case conferences, multidisciplinary meetings and care planning discussions.
  • Build collaborative relationships with external stakeholders and service providers.
Leadership and Supervision
  • Provide guidance, mentoring and clinical supervision to junior social workers, interns and support staff.
  • Support onboarding and training of new staff.
  • Contribute to team discussions and professional development initiatives.
Documentation and Administration
  • Maintain accurate, timely and professional case documentation and reports.
  • Ensure compliance with organisational policies, professional ethics and regulatory requirements.
  • Prepare reports, statistics and service data as required.
Quality Improvement and Service Development
  • Participate in programme planning, service evaluation and quality improvement initiatives.
  • Contribute ideas to improve workflows, client outcomes and service delivery.
  • Support audits and organisational projects where required.
