Job summary

ALLIED SEARCH PTE. LTD. is seeking a Social Work Assistant (CREST) to conduct assessments, plan care with Social Workers, and provide psychosocial support to clients with mental health conditions and/or dementia in Singapore.

The role involves home visits, caregiver support, community activities, and collaboration with SSAs and hospitals to promote CREST services. Training and supervision are provided to maintain professional competencies.

Qualifications

• Diploma qualification in Social Work or equivalent field.
• Experience in mental health or dementia care is an added advantage.

Responsibilities

• Conduct initial assessments and follow-up interventions in accordance with the care plan developed in consultation with the Social Worker.
• Provide case management, home visits, caregiver support and psychoeducation to clients with mental health conditions and/or dementia, as well as their caregivers.
• Deliver psychosocial support through meaningful and client-centred activities, including cognitive stimulation, reminiscence, memory, language and other structured activities based on clients' interests and needs.
• Assist in the planning and implementation of structured preventive activities and programmes for the community, individuals at risk of developing mental health conditions, and existing clients.
• Monitor clients' conditions and progress, and escalate concerns to the Social Worker when appropriate.
• Provide logistical and operational support to clients and their caregivers where required.
• Attend relevant training and supervision sessions to maintain competencies required under the CREST programme.
• Build and maintain partnerships with community stakeholders, including grassroots leaders, Social Service Agencies (SSAs) and hospitals, to raise awareness of mental health and promote CREST services.
• Support the delivery of mental health education and training programmes for members of the public.
• Any other ad hoc duties as assigned.
