Job summary

NTUC Health Co-operative Limited is seeking a dedicated individual to join our integrated care team supporting seniors, including those with dementia. The role focuses on supervising the direct care team, ensuring quality daily living support, and maintaining a safe environment while promoting independence.

You will collaborate with healthcare professionals, manage incident documentation, and help coordinate programmes and staff rosters to sustain high-quality community care services.

Qualifications

• Strong interpersonal and communication skills both verbal and written.
• Be proficient in Google Suite Applications.
• Ability to work independently and as part of a team.

Responsibilities

• Care Delivery: supervise direct care team for ADL tasks, coordinate individual care plans, ensure safe environment, engage with seniors to monitor cognition and function, report concerns to healthcare professionals, implement programmes per guidelines, tailor centre activities, prepare incident documentation.
• Stakeholder Engagement: provide information and guidance to Next-of-Kin and Caregivers.
• Administration & Operations: maintain centre amenities, arrange transportation, monitor inventory, liaise with vendors and facilities, assist in evaluating new equipment, support crisis SOPs, plan staff rosters.
• Quality Management: assist in audits and implement non-clinical SOPs.
• People Management: supervise volunteers, gather feedback, schedule trainings and assessments.
