Job summary

NTUC Health Co-operative Limited seeks a Programme Associate to support the Fun with Friends Programme for seniors, including those with dementia, at Kampung Admiralty in Singapore. You will deliver person-centred activities, coordinate group sessions, and assist in care tasks within a collaborative team.

You will grow in a nurturing environment, plan activities, monitor health status, and provide updates to families.

Qualifications

• Formal healthcare education or equivalent qualification is required or advantageous.
• Experience in social service or healthcare is beneficial.
• Facilitation skills and healthcare knowledge are desirable.

Responsibilities

• Facilitate various multi-modality group activities for persons with dementia as assigned by the programme executive; including exercise sessions, meal preparation groups, cognitive games, music and art-related activities.
• Assist in activity planning and preparation to ensure the smooth execution of the group activities
• Manage clients with different mental, and physical conditions and behaviour of concerns relating to dementia and assist in the monitoring of client’s health status
• Assist in client’s care including providing necessary assistance for transfers, toileting and functional mobility
• Assist in administering assessments to clients and caregivers, perform data collection
• Perform timely documentation for the client post-session
• Assist to provide timely updates to family/NOK of client's engagement
• Support staff or volunteers relating to assist with running the programme
• Assist in inducting new community care associates and provide necessary guidance
• Assist in the procurement of activities materials, with approval sought from RO/Programme Executive
• Perform general housekeeping to the programme premise
