NTUC Health Co-operative Limited in Singapore seeks a seasoned Operations Lead for Home Care to oversee frontline operations in a designated zone, optimize manpower, and ensure high-quality service delivery for seniors and their caregivers.

You will recruit and supervise care staff, coordinate complex referrals, map client journeys, engage with external partners, and drive process improvements across service delivery, ensuring alignment with NTUC Health standards and patient-centred care.

Qualifications

Have at least 3 years of relevant experience in healthcare or community care operations.
Hold a Bachelor’s Degree or a Diploma in Health Science Management, Social Services, or a related discipline.
Strong customer service and communication skills, able to engage effectively with internal and external stakeholders.
Be helpful and resourceful, able to guide next-of-kin through available support and services.
Be proficient in Google Workspace and Microsoft Office applications.
Be fluent in English and at least one additional local language to communicate with elderly clients and families.
Be willing to travel to visit clients as part of care coordination.

Responsibilities

Oversee the allocation and management of care coordinators for home care clients in a geographic area.
Ensure timely follow-up with clients and next-of-kin for onboarding of services.
Handle clients and caregivers and participate in family conferences and case discussions.
Counsel and educate caregivers on providing appropriate home care.
Schedule regular home visits to engage clients and caregivers.
Monitor leave and manpower to maintain service levels.
Recruit and supervise care staff and coordinate onboarding and training.
Coordinate assessments to formulate care plans and monitor progress.
Code information using interRAI and ensure data quality for clinical decisions.
Coordinate within the zone to implement required support for clients and caregivers.
Support complex referral management, service delivery, and escalation of issues.
Engage with local partners to support NTUC Health clients and pilot workflows.
Prepare data and monitor quality improvements; support revenue growth initiatives.
Provide secretariat support for meetings and other duties as assigned.
