Job summary

NTUC Health Co-operative Limited in Singapore is seeking a Community Care Associate to join our dedicated care team. You will work with seniors to promote independence, monitor health, assist with meals, and support care plans in a person-centered model.

Under supervision, you will help with pre-packed medications, health assessments, activities, and basic daily living tasks while building rapport with clients and families in a collaborative environment. Training is provided.

Qualifications

• Minimum WPLN Level 4 in Reading, Listening, and Speaking, and Level 3 in Writing.
• Certificate in Healthcare or related course is preferred.
• Experience in social services or the healthcare industry is preferred.

Responsibilities

• Assist in serving pre-packed medication.
• Assist in client health monitoring and assessment.
• Aid in meal preparation and serving.
• Assist in the implementation and monitoring of the Initiate Care Plan (ICP).
• Provide dysphagia care and EatSafe protocol.
• Support clients in distress including persons living with dementia.
• Assist in the Modified Barthel Index (MBI) assessment.
• Plan and conduct structured and interactive activities.
• Perform basic Activities of Daily Living (ADL).
• Assist with Passive Range of Motion exercises (PROM); implement and support individual therapy.
