Touch Community Services Limited is seeking a Care & Support Executive for an Active Ageing Centre (AAC) to assist in the day-to-day operations, manage inquiries, coordinate befriender volunteer engagement, and maintain records. The role involves outreach to seniors, understanding their care needs and ensuring services are referred timely.

Qualificações

• Diploma in any discipline, preferably with 2-3 years of relevant experience in eldercare or healthcare sector.
• Preferably with experience working with elderly clients in a community setting, especially in healthcare or social service sectors.
• Strong interpersonal and communication skills, the ability to work independently and as part of a team, and good organizational and time management skills.
• A passion for working with seniors and a commitment to promoting active ageing.

Responsabilidades

• Outreach & Engagement: Plan and lead outreach strategies to engage seniors in the community, with emphasis on identifying and connecting with socially isolated seniors.
• Develop and implement targeted approaches (e.g., home visits, neighbourhood outreach, collaborations with grassroots partners) to raise awareness of centre programmes and services.
• Track outreach activities and outcomes to ensure effectiveness and continual improvement.
• Care & Support Services: Provide seniors and their caregivers with in-depth information and guidance on available programmes, services, and resources.
• Conduct needs assessments and make appropriate referrals to community partners, healthcare providers, and social service agencies.
• Ensure follow-up support for seniors and caregivers, monitoring progress and addressing gaps in care.
• Volunteers Management: Recruit, train, and manage a pool of befriender volunteers to support seniors through companionship and care.
• Oversee volunteer engagement strategies, ensuring befrienders are equipped, motivated, and recognised for their contributions.
• Match volunteers to seniors based on needs, strengths, and suitability, monitoring the sustainability of the befriending relationship.
• Operations & Administration: Establish and maintain efficient systems for case recording, volunteer management, and reporting of outreach and care activities.
• Monitor service delivery standards, ensuring compliance with organisational SOPs and funder requirements.
• Contribute to data collection and outcome measurement to evaluate impact and support decision-making.
• People Leadership: Leads AAC officers and volunteers. Provides support, motivates staff and volunteers, ensures well-being of team members at the ground level.
