Job summary

Vanguard Healthcare Pte. Ltd. at Vanguard Active Ageing Centre invites a motivated coordinator to support compliance, oversee day-to-day operations, and drive community outreach for seniors in Pasir Ris/Woodlands.

You will build relationships with seniors, plan activities, manage volunteers, monitor KPIs, and ensure data confidentiality in line with PDPA policy.

Qualifications

• A degree in any discipline.
• 3–4 years' experience in community care setting preferred.
• Good communication and interpersonal skills.
• Can-do attitude with interest to engage seniors.
• Team player, organized and meticulous.
• Eye for detail and event planning.
• Willing to work occasional weekends.
• Proficient in Microsoft Office and data presentation.

Responsibilities

• Support the Manager in overseeing compliance in day-to-day operations, directives, KPI and all work processes.
• Conduct home visits, community outreach and community screening for seniors in the boundary.
• Develop and implement outreach strategies.
• Build and maintain rapport with community seniors.
• Plan and implement activities and programmes to promote active lifestyles and social engagement.
• Gather seniors to join planned programmes.
• Provide support to seniors requiring befriending and track efforts.
• Provide information and refer seniors to appropriate services.
• Recruit, develop and retain volunteers/senior volunteers.
• Assist seniors on vital signs monitoring.
• Compile and submit required reports on time.
• Provide admin support such as documenting purchases, claims and invoices.
• Track expenses and keep within budget.
• Supervisory role to associates and ensure competencies.
• Arrange and track essential courses.
• Attend to walk-in, phone and text enquiries.
• Track AAC KPIs such as outreach and programmes participation.
• Ensure confidentiality of seniors’ data per PDPA policy.

Location: Vanguard Active Ageing Centre (Pasir Ris / Woodlands)

Responsibilities:

• Support the Manager in overseeing compliance in day-to-day operations, directives, KPI and all work processes.

• Develop and implement outreach strategies.
• Build and maintain relationship/ rapport with community seniors.
• Plan and implement activities and programmes based on seniors’ needs and interest to promote active lifestyles and social engagement.
• Gather seniors to join the planned programmes.
• Provide support to seniors who requires befriending and track the efforts done.
• Provide information and refer the community seniors to appropriate services as required.
• Recruit, develop and retain volunteers/ senior volunteers.
• Assist community seniors on vital signs monitoring.
• Compile and submit required reports on a timely basis.
• Provide administrative support such as putting up document for purchases, claims and processing invoice received.
• Track the expenses for the AAC and keep expenses within budget.
• Supervisory role to associates and ensure their competencies.
• Arrange and track essential courses required.
• Any other duties as assigned by the Manager.
• Attend to walk-in, phone and text enquiries and correspondence.
• Track AAC KPIs such as outreach, programmes participation and befriending.
• Ensure confidentiality of the seniors’ data, in line with Organisation’s PDPA policy

Requirements:

• A degree in any discipline.
• 3 to 4 years' experience in community care setting is preferred.
• Good communication and interpersonal skills.
• Can-do positive attitude, with interest to engage the seniors.

• Eye for details and meticulous in planning of events from start to the end
