Job summary

loving heart multi-service centre is seeking a Centre Programme Facilitator to support the daily operations of the Active Ageing Centre, coordinating activities that enhance health, social engagement and well‑being of seniors in the community.

You will conduct outreach to identify isolated or vulnerable seniors, manage volunteers, and prepare reports and evaluations to track outcomes and participation, ensuring alignment with funders' objectives and LH's mission.

Qualifications

• 1 year of relevant experience in programme facilitation or operational duties.
• Proficiency in MS Office 365 and basic data reporting.
• Strong interpersonal and verbal communication skills.

• Plan, coordinate, and deliver activities and services for seniors' health and well-being.

Reporting to the Centre Supervisor, you will work collaboratively to support the daily operations of the assigned Active Ageing Centre (AAC), ensuring its activities and services align with the objectives set by funders and Loving Heart (LH).

Responsibilities:

• Responsible for planning, coordinating, and delivering activities and services that support the health, social engagement, and well-being of seniors in the community.
• Conduct outreach to identify socially isolated or vulnerable seniors
• Manage volunteers in delivering activities and services.
• Prepare reports and programme evaluations
• Track programme outcomes and participation rates.

Requirements:

• At least 1 year of relevant experience in programme facilitation, operational duties.
• WSQ qualification & above.
• Comfortable with MS Office 365.
• Good interpersonal and communication skills
