Qualifications

Responsibilities

• Support coordination and management of patients receiving care at home
• Conduct pre-admission screening and assist with developing patient care plans
• Monitor patients’ general condition, progress and vital signs
• Escalate abnormal findings or care concerns to nurses / clinicians
• Handle patient and caregiver enquiries and provide care plan updates
• Support telehealth monitoring and basic patient education
• Maintain accurate patient records and documentation
• Coordinate with healthcare providers and community services for continuity of care
• Ensure necessary equipment and supplies are available for patients
