Job summary

THE SUPREME HR ADVISORY PTE. LTD. is seeking a Nursing Care Coordinator in Singapore to coordinate referrals, contact families, and schedule care teams. You will liaise with hospitals, clients and caregivers to initiate services and ensure smooth operations.

The role requires coordinating staff schedules, managing last-minute changes and maintaining daily client care records, with a Monday–Friday schedule and onsite location at Tagore Lane.

Qualifications

• Diploma or relevant Healthcare, Nursing, Community Care, Social Services or Healthcare Administration required.
• Experience coordinating staff schedules and handling last-minute changes.
• Excellent communication with clients, families, hospitals and care teams.

Responsibilities

• Coordinate and manage IC referrals from hospitals.
• Contact clients and families to arrange the start of care services.
• Coordinate Nursing Aide/Caregiver assignments and schedules.
• Monitor daily attendance and ongoing client services.
• Arrange replacement staff when necessary.
• Communicate regularly with clients, families, hospitals and the IC team.
• Monitor clients' service periods and upcoming discharge dates.
• Assist on simple admin task.
