Job summary

RECRUIT EXPRESS PTE LTD is seeking a Service Coordinator for Home Care services based in AMK, Singapore. The role focuses on client-centered care coordination, discharge planning, monitoring and caregiver education, to ensure smooth service delivery.

You will handle walk-in, online and phone inquiries, assess client needs with screening tools, promote suitable health service packages, and collaborate with internal teams and external partners on referrals while ensuring timely follow-up.

Qualifications

• Min Diploma or Advanced Diploma in healthcare-related discipline or Bachelor Degree in related field.

Responsibilities

• Provide client-centric care coordination and discharge planning for Home Care clients.
• Manage walk-in, online and phone enquiries on Health services.
• Screen clients to identify needs and promote suitable Health service packages.
• Collaborate with divisions and external partners on referrals and follow-up.
• Administer coordination of services requested and document records.
• Schedule appointments for Home Care services.
• Conduct home visits and monthly telephonic follow-ups with clients and caregivers.
• Collaborate to recommend care programs and assist with appeals.
• Educate caregivers and assist with reassessment of care plans every six months.
• Work with Operations and Nurse Manager on discharging clients.
• Participate in program development to meet evolving eldercare needs.
• Identify opportunities to drive service utilization and revenue growth.
