Job summary

RECRUIT EXPRESS PTE LTD is seeking a care coordinator to perform pre-admission screening and provide holistic home-based care for patients and caregivers in the West Singapore.

You will collaborate with nurses, monitor patients’ condition, identify care gaps, document progress, develop care plans, and coordinate referrals. Telehealth monitoring and basic patient education are also part of the role.

Qualifications

• Pre-admission screening and home care support for patients and caregivers.
• Collaborate with nurses to monitor and provide holistic home care.
• Identify deviations, care gaps, and report to team.
• Document patient progress and maintain records.
• Coordinate with community resources for referrals.
• Develop care plans based on assessed needs.
• Support telehealth monitoring and basic patient education.

Responsibilities

• Monitor vital signs and escalate red flags to nurses/physicians.
• Provide basic patient education and health coaching.
• Ensure equipment and supplies for patient care are available.
• Participate in quality improvement, risk management, and ad-hoc duties.
• Maintain accurate documentation of patient progress and treatment plans.
