Alexandra Hospital is seeking a Social Work Coordinator to assist in administrative and discharge planning activities and to support Medical Social Workers in casework. You will manage the department counter, interact with patients and families, conduct financial assessments, and facilitate referrals.

The role reports to a Medical Social Worker and collaborates with the Team Leader for Social Work Coordinators.

Qualifications

A Diploma in Social Work or related fields is required or preferred.
The role reports to a Medical Social Worker and works with the Team Leader for Social Work Coordinators.
Strong interpersonal and problem-solving skills are necessary.
IT knowledge and basic computer skills are necessary for the job.

Responsabilités

Provide administrative and operational support for the department, including managing the counter and related duties.
Direct work with patients and families, including intake interviews and financial assessments, and assist with discharge planning.
Support Medical Social Workers in case management through translation and patient escort/home visit support.
Participate in work process improvements and quality improvement projects.
Collaborate with other hospital departments and external organizations to facilitate patient care and experience.
Assist the department in programmes and projects.
