Alexandra Hospital, Singapore is seeking a Social Work Coordinator to support financial assistance and discharge planning for patients. The role entails direct work with patients and families, intake interviews, and collaboration with Medical Social Workers and the Team Leader for Social Work Coordinators.

The position requires a Diploma in Social Work or related field, strong interpersonal and problem-solving abilities, and IT literacy.

Qualifications

Diploma qualification required; Diploma in Social Work or related fields preferred.
Role reports to Medical Social Worker and collaborates with Team Leader for Social Work Coordinators.
Good interpersonal and problem-solving skills are necessary; IT knowledge beneficial.

Responsabilités

Provide administrative and operational support within the department, including counter duties.
Engage directly with patients and families for intake interviews, assess financial needs, arrange assistance and referrals for discharge planning.
Assist Medical Social Workers with case management, translation, patient escort and home visit support.
Participate in process and quality improvement projects.
Collaborate with other hospital departments and external organizations to enhance patient care and experience.
Support department programmes and projects.
