Job summary

Alexandra Hospital Singapore is seeking Social Work Coordinators to support financial assistance and discharge planning for patients. You will assist in intake interviews, financial assessments, referrals, and coordinate administrative duties to keep the department running smoothly.

Qualifications

A Diploma is required. A Diploma in Social Work or related fields preferred.
The role reports to a Medical Social Worker and works with the Team Leader for Social Work Coordinators.
Good interpersonal and problem-solving skills are necessary.
IT knowledge and skills are necessary for the job.

Responsibilities

Provide support for administrative and operational running of the department, including counter duties.
Direct work with patients and families including intake interviews and financial assessment.
Assist with referrals to facilitate discharge planning in selected care areas.
Support Medical Social Workers in case management, translation, patient escort and home visits.
Participate in process improvements and quality improvement projects.
Collaborate with other departments and external organizations to facilitate patient care.
