You will handle counter duties, intake interviews, and financial assessments to facilitate patient referrals and discharge planning in selective care areas.

The role involves assisting Medical Social Workers in case management, translation, patient escort, and home visit support, with opportunities to contribute to process and quality improvements

Qualifications

• Diploma required; Diploma in Social Work or related fields preferred.
• Role reports to Medical Social Worker and collaborates with Team Leader for Social Work Coordinators.
• Good interpersonal and problem-solving skills are necessary.
• IT knowledge and skills are necessary for the job.

Responsabilités

• Provide administrative and operational support for the department, including managing the counter and related duties.
• Conduct intake interviews, perform financial assessments, and assist with referrals to facilitate discharge planning in select care areas.
• Assist Medical Social Workers in case management with translation, patient escort, and home visit support.
• Participate in work process improvements and quality improvement projects.
• Collaborate with other hospital departments and external organizations to support patient care and experience.
• Support the department in programmes and projects.
