Job summary

DEMENTIA SINGAPORE LTD. in Singapore is seeking a qualified social worker to assess cases, develop individualized care plans for persons with dementia and their carers, and provide psychoeducation and supportive counselling.

You will liaise with multidisciplinary teams, document case recordings, and support programme evaluation. The role involves guiding caregiver support groups, facilitating outreach events, and collaborating with external service providers.

Qualifications

• Degree or post-graduate diploma in social work, psychology or counselling.
• Preferably 2 years of healthcare/community care clinical experience.
• Good oral, written and interpersonal communications skills.
• Able to work on occasional Saturdays and weekday evenings.
• Prior working experience in dementia care will be an added advantage.

Responsibilities

• Conduct case assessment and formulate Individualized Care Plan for Persons with Dementia and their carers.
• Provide intervention such as psychoeducation, supportive counselling and service linkages.
• Liaise and collaborate with multidisciplinary professionals and external service providers for holistic care management.
• Provide case recordings and relevant documentations.
• Participate in team meetings and case conferences as required.
• Assist in the preparation of data and statistics for programme evaluation and reporting purposes.
• Plan and facilitate Caregiver Support Group sessions.
• Plan, support and facilitate outreach events and talks.
• Any ad-hoc tasks as and when required.
