Job summary

The Supreme HR Advisory Pte. Ltd. is seeking a qualified Social Worker to join our multidisciplinary care team in Singapore.

You will assist with intake, assessments and admissions, provide psychosocial support to residents and families, and coordinate referrals to community resources and government schemes. Responsibilities also include Advance Care Planning, discharge planning, and bereavement support, with emphasis on elderly and dementia care.

Qualifications

• Bachelor degree in Social Work or related field plus SWAAB registration.
• 3–5 years of relevant experience, preferably in hospital or community setting.
• Experience with elderly and dementia populations.

Responsibilities

• Intake, assessment and admission support for prospective residents/clients.
• Provide counselling and psychosocial interventions and coordinate referrals.
• Conduct Advance Care Planning discussions and document decisions.
• Develop discharge and transition plans with multidisciplinary team.
• Coordinate respite care, palliative care and bereavement support.
• Perform risk assessments and crisis intervention as needed.
• Maintain records and documentation; participate in quality initiatives.
• Collaborate with healthcare institutions and community services.
